Stars Assistance Form

The Stars Assistance Form allows faculty and staff to listen for student challenges and refer a student who may be experiencing academic or non-academic concerns. Visit Student Care Network for details on support services available.

Students can use the Stars Assistance Form to ask for help for themselves, report incidents, or refer a friend who needs support.

Once a referral is submitted, professional staff member from the Student Care Team will reach out to offer support and connect you to resources on campus. Simply complete and submit the referral form. You will be notified regarding receipt of your referral. Referrals and information submitted is handled with care.
