If you have a documented disability or believe you have a disability but don’t yet have documentation, you may request accommodations for SOAR and/or for the academic year.  Our Accommodations & Disability Access (ADA) office will follow up with you to coordinate support with the SOAR team.

Examples of some possible SOAR accommodations: 

  • Accessibility or mobility needs  

  • Testing accommodations, such as extended time on math placement exam 

  • Sign language interpreter 

  • Alternate format of materials  

  • ADA accessible room 

  • Proximity to restroom 

  • Access to a refrigerator in housing 

  • Access to a lactation room

  • Access to a prayer room

What's Next?

To request accommodations, follow these steps.

  1. Open this Confidential Request for Accommodations Form

  1. Download and save the form on a computer 

  1. Fill out the form (it is a digital form, so you can type your answers and signature) 

  1. Save the completed form on your computer 

  1. Email ada@dom.edu and attach the completed form to your email; also include a few times you could meet over the next week

You will not be asked to provide documentation for SOAR accommodations.

ADA will work with you to request the necessary documentation needed to continue your accommodations at Dominican.
